© Copyright 2011 Guy’s Academy Hair, Skin & NailsSTUDENT RIGHTS AND RESPONSIBILITIESGuy’s Academy students have the right to know:names of accrediting and licensing agenciesprograms, facilities and facultycost of attendingrefund policy for student who drops prior to completing the programfinancial assistance available from Federal, State, Local, Private and Institutional programshow the Financial Aid Administrator determines the student’s financial need and eligibilityeach type of aid to be received and how it will be disbursedhow to submit appeals under various school policiesmethod of determining satisfactory academic progress and how it affects the student’s financial aid            eligibilityterms of all loans borrowed by the student including interest rates, repayment amounts, cancellation and            deferment            provisionscriteria for continued eligibility for financial aidfacilities and services available to disabled studentsplacement types and placement ratescompletion rateretention ratestudent body diversityGuy’s Academy students have the following responsibilities:review and consider all aspects of programs before enrollingaccurately and truthfully complete financial aid and enrollment applicationsprovide additional supporting documentation, verification, necessary corrections, etc. as requested by           Guy’s  Academy or affiliated agenciesread, understand and keep copies of all forms receivednotify Guy’s Academy of any change in their financial circumstances, name, address, and contact            informationunderstand satisfactory academic progress policyunderstand refund policiessign all required certification statementsrepay all Direct Stafford Loans according to the terms and conditions of signed Master Promissory NotesCONSUMER RIGHT-TO-KNOWConsumer information is computed as a percentage for all programs offered at Guy’s Academy for first-time, full-time students who enrolled from August 1, 2010  to October 31, 2010 and completed the program of study within 150% of the normal time frame. First-time students are defined as students that enroll in post-secondary training for the first time beyond the high school level.
